Special Patient Note Service – Request New User
Please fill out the below form for access to the Adastra Special Patient Note Service (Web Access).
Please note, the practice manager will be required to authorise the request by responding to an automated email that will be sent to the address you provide below, you will also receive a copy of this email for your reference.
Please note: generic accounts are NOT allowed, please request an account for each individual person you would like setting up. Non-clinicians can also have an account and update notes on behalf of clinicians.
Requests can take up to 5 working days to be processed.
